This calculator determines the Fluconazole dose for treating fungal infections, using a concentration of Fluconazole 50 mg/5 mL (10 mg/mL Fluconazole), based on the patient’s weight and condition:
Dose in mL and mg (based on Fluconazole):
Oropharyngeal Candidiasis: 3 mg/kg po once daily
Esophageal Candidiasis: 3 to 6 mg/kg po once daily
Systemic Candidiasis (Candidemia and Disseminated Candidal Infections): 6 to 12 mg/kg po once daily
Cryptococcal Meningitis: 6 to 12 mg/kg po once daily
Adults: 100 to 400 mg po once daily (up to 800 mg for invasive systemic fungal infections); Uncomplicated vulvovaginal candidiasis: 150 mg po as a single dose
Where:
Administer as prescribed for fungal infections. ⚠️ Always consult a healthcare provider.
Enter the patient’s weight in kilograms (kg) or pounds (lb) with precision up to 0.001 units, select the weight unit from the dropdown, choose the condition from the dropdown, and click "Calculate." The calculator provides the dose in mL and mg (Fluconazole) for pediatrics (>1 month) and adults, including loading doses where applicable.
Example: Weight: 10 kg, Systemic Candidiasis:
Pediatric > 1 month: Min Dose: 6 ml (60 mg) po once daily Max Dose: 12 ml (120 mg) po once daily Loading Dose: Administration of a loading dose on the first day of treatment, consisting of twice the usual daily dose, results in plasma concentrations close to steady state by the second day (Max 12 mg/kg or 800 mg/day) Adult Dose: 100 to 400 mg po once daily (up to 800 mg for invasive systemic fungal infections) Uncomplicated vulvovaginal candidiasis: 150 mg po as a single dose Calculations must be re-checked and should not be used alone...
Consult a healthcare provider before administering.
